Required notice under Texas Health & Safety Code §181.105

Requesting your Health Care Records

You have the right to request a copy of your health care records. Email Your Request To:

Please use “Health Records Request” as the subject line and include the following information:

• Your full name
• Your telephone number
• Your date of birth
• Your current email address
• The mailing or email address where you would like the records sent
• A description of the specific records you are requesting

Please do not include detailed medical information in the email subject line. After receiving your request, your practitioner may contact you if additional information is needed to verify your identity or process the request.
